What are the Dhadkan Zindagi Kii Episode 28 medical cases?

How does Dhadkan Zindagi Kii Episode 28 medical cases begin?

This episode starts with a conversation between the girl in the picture who is a patient and a young doctor taking history. The patient provides a history of less sleep. She appears to be a celebrity.
My reaction to this patient can be seen in the below video. This brings an important question about sleep. Sleep and rest and critical for a healthy lifestyle and you should make it a priority.

 

What happens next to the patient with pain abdomen?

This patient has tests done like a EUG. I am not sure what EUG is. The patient complains of pain abdomen and vomiting with a history of acidity. I presume EUG is an endoscopy. Endoscopy is the passing of a tube into the mouth to look at the parts of the gastrointestinal tract; esophagus, stomach, and small intestine. This is frequently done under anesthesia or sedation since passing an endoscope can be uncomfortable. This patient was found to have an ulcer in the duodenum.

As a side note, using abbreviations can be a source of errors. I encourage young Doctors not to use any abbreviations but make sure it is spelled out to ensure everyone is clear about it. Just my take on this.

What other tests are done?

The doctor also decides to check an X-Ray to look for air under the diaphragm. This is an old test and the presence of air under the diaphragm indicates a perforated ulcer. In simpler terms, this means, the ulcer has a hole, and the gas normally present in the intestines has leaked out and is showing as air under an organ called the diaphragm. The diaphragm separates the chest from the abdomen.

More recently, a CT scan is performed to locate the perforation.

Does the patient with an ulcer require surgery?

Dhadkan Zindagi Kii Episode 28 medical case moves on to say that she does require surgery. A discussion occurs between the patient and the doctor. The doctor explains that it is major surgery. The surgeries are classified in one way into

  • Low-risk surgery
  • Moderate risk surgery
  • High-risk surgery

I personally think this should be more of an intermediate surgery. This is not easy to say without knowing the full history. This may well be high risk and not possible to find out until the end.

The episode goes on to say that the surgery is already over. They do not show anything in the operation theater. It appears that the surgery went without any problems.

Dr. Deepika runs over the postoperative instructions to Dr. Abhay. She specifies nothing to feed through the mouth and place a Ryle’s tube. This is a standard procedure after an operation for a perforated ulcer. The nutrition during this time goes through an intravenous line. A Ryle’s tube is a tube placed in the mouth or the nose into the stomach. This tube helps suction secretions from the stomach.

What happens next with the Dhadkan Zindagi Kii Episode 28 Medical Cases?

Well, this patient who had surgery, complains of chest pain going to the left hand. Chest pain after surgery especially this type moving to the left hand can be a dangerous sign of an underlying heart problem. In this case, tests like electrocardiogram (ECG), echocardiogram, and blood tests are done to test for a heart attack.

This patient is actually shown to have a heart attack. Dr. Abhay starts treating the chest pain as a heart attack using morphine and he mentions using nitroglycerine using non-adsorptive tubing.

Nitroglycerine is a medicine classified as a vasodilator that is used in the treatment of heart attack. This medicine helps increase blood flow into the heart which is important to prevent and treat a heart attack. This medicine can get stuck on the tubing and may not get into the patient. Hence, it is used with special non-adsorptive tubing.

What is the second medical case in Dhadkan Zindagi Kii Episode 28?

The second medical case is a prisoner from the jail Mehul Ramsay. He seems to cheat people which has landed him in jail. He presents with vomiting. The doctor’s team enquire the police accompanying Mehul about the vomit color. The color of the vomit can many times provide a clue to the diagnosis. The police and Mehul himself deny knowing the vomit color.

What happens next to the second vomiting case in Dhadkan Zindagi Kii Episode 28?

The team finds out Mehul has a history of eating metal and stones. This condition is called Bezoar. This is an unusual condition where patients eat things that they are not supposed to eat. This patient requires surgery. This is a situation where the stomach is cut open and things are taken out.

There are many types of bezoar. I have not personally seen a patient with the habit of eating stones or metal. I have personally seen a type of bezoar called Trichobezoar. In this case, patients pull their hair and eat it. Once it gets too much it blocks the stomach (obstruction) and causes symptoms like vomiting.

The show creators have done research on rare interesting cases.

What is the third medical case in Dhadkan Zindagi Kii Episode 28?

The third case is not shown. But the discussion goes on to say by Dr. Siya that the patient has fasciitis of the perineum, a condition known as Fournier’s gangrene.

Fournier’s gangrene is a rare condition that results in gangrene of the male parts. The penis, scrotum, testicles get gangrenous and there is a risk that the patient can lose these parts. This is an emergency and has to be treated immediately with surgical debridement and broad-spectrum intravenous antibiotics.

Who gets surgery next?

The hospital ends up not having free operation theater to treat Fournier’s gangrene and laparotomy for Mehul. The surgeons Dr. Siya and Dr. Ansari end up fighting whose case has to go first.

The episode ends by saying both the patients are doing fine. No specifics of the operation or anything in the operation theater is shown.
